• AKC Artificial Insemination Using Fresh Extended Semen
• AKC Artificial Insemination Using Fresh Semen
• AKC Artificial Insemination Using Frozen Semen
• AKC Litter Registration Application
• Chilled Semen Shipment & Credit Card Authorization (These forms are to be completed by the bitch’s owner BEFORE the semen will be shipped.)
• Frozen Semen Disposition (This form is to be completed by the semen owner to give authorization to release frozen semen for a breeding, transfer ownership of semen, or transfer semen to another storage facility.)
• Frozen Semen Shipment (The following two forms must be completed.)
  • Frozen Semen Disposition (This form is to be completed by the stud’s owner BEFORE the semen will be released.)
  • Frozen Semen Shipment & Credit Card Information (These forms are to be completed by the bitch’s owner BEFORE the semen will be shipped.)
• Ovulation Timing (This form is be be filled out and presented before or at time of your appointment.)
• Ovulation Timing & Artificial Insemination Article (This article written by Dr. Kampschmidt explains the ovulation timing process and artificial insemination options.)
• Record of Collection Ownership (Please complete form before collection takes place. All stud dog owner(s) listed on the Registration Certificate have to release the collection to the new owner.)
• Semen Freeze Packet (Complete all forms below.)
  • (1:6) AKC Frozen Semen Collection Statement (This Frozen Semen Packet form is to be completed by the stud’s owner.)
  • (2:6) Frozen Semen Packet (This form is to be completed by the stud’s owner.)
  • (3:6) Record of Collection Ownership (All stud owners listed on the Registration Certificate have to release the collection to the new owner.)
  • (4:6) Stud Health History Record (This form is to be completed by the stud’s owner.)
    (5:6) Disposition Form In Case of Death (This form is to be completed by the stud’s owner.)
    (6:6) Semen Storage Credit Card Authorization Form (This is to be completed by the stud’s owner.)
• Semen Storage Discontinuation  (Use this form to discontinue semen storage service.)
